Annual School Report
Member schools should use the links below to assist with annual reporting. Schools should contact with any questions regarding the LMA Annual School Report Form. There is also a link to the LDOE Nonpublic School Annual Report User Guide.
All member schools must participate in a LMA visitation at least once every 5 years in accordance with certification standards. At the time of the visitation recommendations are made regarding whether a school is in good standing, with any needed improvements discussed. Improvements must be made in order to receive certification approval status. Anytime a new program is added to a member school or a new facility is open, a visitation must be completed to remain in good standing.
