Annual School Report

Member schools should use the links below to assist with annual reporting. Schools should contact lma@network.net with any questions regarding the LMA Annual School Report Form. There is also a link to the LDOE Nonpublic School Annual Report User Guide.

All member schools must participate in a LMA visitation at least once every 5 years in accordance with certification standards. At the time of the visitation recommendations are made regarding whether a school is in good standing, with any needed improvements discussed. Improvements must be made in order to receive certification approval status. Anytime a new program is added to a member school or a new facility is open, a visitation must be completed to remain in good standing. Schools should use the below links to review in preparation for visitations. Direct all questions to lma@network.net.
